Medical Social Work Services, Inc.

The organization and mission of Medical Social Work Services, Inc. is the product of extensive experience* in the Medicare Home Health field. 

With the Federal Home Health Law as our guide and direct services as our laboratory, we developed a model program for delivering social services maximizing the potential for both recovery and continued independence.

*Nearly 10,000 Medicare Home Health care plans for 27 Medicare Home Health Agencies, since 1990                                    

Service Area

Our direct services are focused in  Martin, St. Lucie, and Indian River counties on Florida's Atlantic coast, although in special needs situations, we provide services in the adjoining counties of Palm Beach, Okeechobee, and Brevard.  (To see Coverage Area < click)

We do not intend to expand direct services to other areas because the effectiveness of care planning  is directly related to knowledge of the community, as well as a history of successful working relationships.  These are qualities that develop over time from living in and being a part of the community.

Connection with Community

Our medical social workers have provided health care and social services in this area for a combined total of more than 100 years, which enhances our ability to access community resources, develop comprehensive care plans, and insure  continuity of care.  (For more information about experience in and connection with our community click > About Us)

Model Program

Beginning in 1990, the Director of Medical Social Work Services, Inc. first encountered barriers to effective Medicare Home Health social service delivery. One by one, solutions were devised to eliminate barriers. This process eventually led to the development of a program  model  producing the results as envisioned in the Federal Medicare Home Health Law. (For more information on Model Program < click)

Our approach enables elderly patients to be optimally independent. The depth and breadth of training, experience, and knowledge necessary to assess needs and to incorporate all available resources into a quality plan of care as expected by the Federal Home Health Law, requires the support of a social services agency.  This premise is underscored by the logistical problems inherent in home based services, fluctuations in referral volume, and the extraordinary broad scope of information required for successful navigation of our social service and health care bureaucracies.
