Care Plan Development
We plan and
establish services allowing our clients to continue living at
home for as long as possible, at the highest level of
independence, and in an environment conducive to preservation
of dignity and quality of life.. Should living at home
cease to be an option, we will research all available
resources and recommend the most appropriate alternative in
the least restrictive environment.

Scope of Services
We will assist
in planning for immediate and long term needs, in the
selection and integration of services most suited to
meet needs, and in ongoing oversight and coordination of those
needs, if necessary. We will insure the coordination of
medical care, identify and address safety issues, arrange for
support services necessary to maximize self-sufficiency and
maintain dignity. We will provide guidance for
financial, legal, and insurance issues, in consideration
current and projected needs, resources, and desires of both
the elder and their caregivers.
