Care Plan Development

We plan and establish services allowing our clients to continue living at home for as long as possible, at the highest level of independence, and in an environment conducive to preservation of dignity and quality of life..  Should living at home cease to be an option, we will research all available resources and recommend the most appropriate alternative in the least restrictive environment.

    • A comprehensive assessment (identifies needs)

    • An initial plan of care presented in conceptual terms to be reviewed, revised, and approved by the elder and caregivers (identifies the type of  services necessary to address needs).  This report becomes a "blueprint" for the selection of the most appropriate and cost effective combination of available resources. 

    • An operational plan of care with specific recommendations for implementation (sets priorities, identifies service providers, establishes reporting process for feedback,  and presents a plan for eliminating the need for ongoing MSWS "management" services).

Scope of Services

We will assist in planning for immediate and long term needs, in the selection and integration of services most suited  to meet needs, and in ongoing oversight and coordination of those needs, if necessary.  We will insure the coordination of medical care, identify and address safety issues, arrange for support services necessary to maximize self-sufficiency and maintain dignity.  We will provide guidance for financial, legal, and insurance issues, in consideration current and projected needs, resources, and desires of both the elder and their caregivers.
